PACE, which stands for Program of All-inclusive Care for the Elderly, is a nationally recognized model of care funded by Medicare and Medicaid. CarePartners PACE specifically serves individuals within Buncombe, Henderson, McDowell, and Transylvania counties (excluding zip codes 28774 and 28749) who meet the North Carolina Nursing Home Level of Care Criteria but are able to live safely at home with the support of PACE services.
The cornerstone of CarePartners PACE is its all-inclusive approach to care. This means that the program provides a wide array of medical and support services, all coordinated by an interdisciplinary team of professionals. This team works collaboratively to develop an individualized care plan tailored to each participant’s specific needs. With the exception of emergency services, all healthcare services must be authorized by CarePartners PACE to ensure coordinated and comprehensive care. Unauthorized services may result in full financial liability for the participant.
CarePartners PACE offers a comprehensive suite of services, including but not limited to:
- Medical Care: Primary care services encompassing doctor and nursing care, specialist medical services, prescription drugs, laboratory and X-ray services, and hospital care.
- Therapeutic Services: Physical therapy, occupational therapy, recreational therapy, and nutritional counseling to support physical and functional well-being.
- Personal Care & Support: Home care services to assist with daily living activities, social services, and social work counseling to address psychosocial needs.
- Other Essential Services: Dentistry, emergency services, meals, medical transportation, and nursing home services when necessary.
