The Community Care Program is a statewide initiative designed to bridge the gap between Maine residents and vital health programs, social services, and community resources. This program is available to everyone in Maine, with a special focus on individuals who may face obstacles in accessing the support they need and are eligible for.

To begin your journey towards accessing a wide range of support services, we encourage you to fill out our referral form. This simple step can connect you to resources that include, but are not limited to:
- Services that are respectful of and tailored to diverse cultures and languages.
- Resources specifically designed to support mothers, infants, and children.
- Guidance and assistance in navigating the often complex healthcare system.
- Education and resources focused on improving overall health and wellness.
- Options for accessing food assistance, heating support, and transportation solutions.
- Support systems for managing stress and anxiety.
- Up-to-date information and resources related to COVID-19.
We kindly request that you submit one referral per household to ensure efficient processing.
How the Program Works
Once you submit the referral form, our dedicated staff will carefully review your information and connect you with a partner organization best suited to discuss your specific needs and available options. In most cases, you can expect to be contacted by one of our partner organizations within 1 to 3 business days. It is important to remember that each program maintains its own eligibility criteria, and the availability of certain resources may vary depending on your location within Maine.
Important Note: Not for Emergencies
Please be aware that the Community Care Program is not designed to provide immediate access to emergency resources or services. If you are experiencing a medical emergency or urgent health situation, it is crucial to contact your healthcare provider directly, dial 911, or for mental health emergencies, call 988.
Privacy and Confidentiality
Your privacy is paramount. The information you share will only be disclosed to local partner organizations directly involved in addressing your request. Your information will not be shared with federal government agencies or immigration authorities. Personal information shared with the Maine CDC will be securely deleted within 90 days, or as soon as it is no longer necessary to process your referral, whichever period is longer.
